Apply For A Getaway
HOMES OF HOPE only accepts online applications for getaway attendance. After we receive your application it will go through our review process and we will inform you if your family has been selected this year. Your application will stay on file in the event that another family needs to cancel.

Under 21?
The patient or guardian of the patient if under 21, must have a valid driver license ready to upload during the application process.
Release and Liability
We are not responsible for injury of patient or family members who attend a getaway. Please print and sign the following Release and Liability Waiver and have it ready to submit during the application process.
Medical Clearance
Print and have your physician’s office complete the Medical Clearance Form. Please have it available for upload on the application form.
